Health Form

The newly revised LHSON Health Form is required of all students entering the Nursing School in Dallas. This form will be made available during Online Orientation. Included on this form are required immunizations, as well as a physical examination. All Students MUST have a Physical!! Immunizations can be obtained from your choice of location/physician. Once you have a BUMC ID, you qualify to use the BUMC Employee Health Center.
